Education - Graduate from an accredited dental school (candidates must possess either a D.D.S. or D.M.D. degree) Experience - Five (5) years of experience working as a Dentist in a clinical setting and including three (3) years of management experience. Never been disciplined by any professional or peer review organization, governmental agency, licensing body or other applicable entity. Working with culturally diverse limited English or non-English, speaking patients preferred. Fluency in some A/PI languages preferred. Other Requirement(s) - License to practice in Washington State without restriction, probation, or limiting condition or threat thereof. Current and active DEA Registration licensure. Current Washington State Driver’s license.
