Avenue A Family Dental 173 Avenue A, Turners Falls, MA 01376 413-863-2296 Job proposal Position: Dental Hygienist – Full time, Part time Starting Date: TBD Employment Status: Full time (4 days/week), Part time (2-3 days/week) Pay: Up to $60 Minimum 2 days a week. Minimum commitment is 3 months Benefits: 401k with match up to 4%- eligibility starts after 1 year and a minimum of 1000 hours worked during the year. Vacation: 2 weeks (based on your average weekly hours). To qualify you must work at least a minimum of 800 hours a year. Holiday pay: payable if falls on your regularly scheduled work day. Six paid holidays per year. Eligible after 90 days probation. Sick pay: up to 40 paid hours per calendar year. Sick pay are accrued 1 sick pay hour per 30 hours worked. Available after 90 days probation. Must submit an absence form to get reimbursed. Dental benefits (free & discounted procedures) for employee and 50% off for immediate family members (spouse and children) – must have prior approval before services for major procedures. Services provided is up to the discretion of the provider. Fees will be discussed & collected prior to treatment. Annual OSHA HIPPA CPR course reimbursement for the tuition fees. Must submit receipt to get reimbursed. Job Responsibilities Focus on patient care and provide the best dental care experience possible. Work with front office on the hygiene schedules and the daily/ weekly/ monthly goals. Help doctor on duty with treatment and treatment planning. Maintain your work space and the instruments and equipment on a regular basis. Help the team with daily/ weekly/ monthly cleaning. Help create fun and productive work environment. Willing to be a team player and to constantly seek opportunity to improve. Responsible for your own license and the necessary CE for future license renewall.
